Annual Service Fee
The annual fee is $2000 per patient with a maximum of $5000 per household/family (monthly, quarterly, and semiannual payment options are available). You should be able to use your HSA and/or FSA for payment. You also may be able to apply the fee to your insurance deductible. Because all plans are different, please check with your insurance and savings plans to verify.
